Fulltime, exempt position to work as part of a team to manage thesocial service programs of CentraCare in accordance with current existing federal, state, and local standards, as well as our established policies and procedures. Our Social Workersconduct initial and periodic assessments and develops a specific plan of care for the needs of CentraCare participants. Facilitating enrollments and disenrollments from CentraCare and coordinating enrollments and discharge planning to/from hospitals and long term care facilites would be required. Must be comfortable in conducting home visits and assessments. Experience in using an EMR for documentation is required.
