As the TCU Manager and your team will incorporate the skills of a nurse, care manager, and patient advocate.
The Transitional Care Manager will have an active engagement with family caregivers.

Qualifications

TheTransitional Care Unit Managerwe are seeking, must have the following qualifications:

  • Registered Nurse (RN) in the state of Illinois, in good standing
  • Has experience working in a nursing home as a manager of a Transitional Care Unit
  • Ability to teach others, and be a good mentor.
  • Have patience and understanding foryour residents as well as family members of the resident.
