HEALTHCARE COORDINATOR - Manhattan, NY Job Description: The primary function of the Healthcare Coordinator is to provide coordination of medical care for clients. The Healthcare Coordinator will provide services relating to the promotion of wellness for clients through educational lectures, individual counseling and special topic seminars. The Healthcare Coordinator is a member of the interdisciplinary clinical team at their assigned facility and participates in case conferences, treatment and discharge planning meetings. The Healthcare Coordinator serves as a liaison between the agencys clinical treatment team and the external primary medical care provider ensuring continuity of care for clients with medical conditions. Healthcare Coordinator Required Education & Qualifications: Graduate of accredited school of nursing: Bachelors Degree preferred. Current New York State license: Licensed Practical Nurse or Registered Nurse. Minimum of three (3) years nursing experience, in residential or ambulatory health care or behavioral healthcare settings. Basic keyboard skills and computer literacy (Window based environment knowledge preferred). Certification as a CPR/First Aid trainer preferred. Hours: Full Time
