Case Manager – Intensive Case Management– Full time position in Willimantic working with adults with serious and prolonged mental illness. Play a critical role in functional and resource assessment, creating service plans, and assisting clients in skill development. Work in collaboration with other providers within the mental health and social service systems. Assess client needs, provide counseling, assist clients w/developing & managing community service plans. Use of own car required. Bilingual Spanish English fluency preferred.

Minimum Requirements

Bachelors Degree in relevant human service field required.

Salary Range

$14.26 – $16.13 per hour depending on experience
