Care Transition Coordinator/Full time/SGAH Hospital

Associate or higher college degree required.
Must have marketing experience in Home Health or Home Care.
Two to three years work experience in home care or other health care field.
Proficient in PC skills such as the Windows work environment, the internet and using e-mail programs.
Proficient in Microsoft Word, Excel, PowerPoint.
Ability to work independently and handle multiple tasks at one time.
Strong customer service skills on the telephone and in face to face contact.
Strong organizational skills.

The Care Transition Coordinator will develop and maintain referral relationships at Shady Grove Adventist Hosptial to achieve on average eight new referrals per day with an average capture rate of 80 percent. Will complete accurate and timely clinical assessments of potential patients to recommend our various services. Educates referral sources, patients, families, and physicians on home care services, insurance criteria and qualifications. Analyzes referral trends and makes recommendations for new business opportunities. Completes accurate referral in order to facilitate achieving on average six or more admissions per day. Demonstrates behaviors that facilitate open communication, teamwork and collaboration. Handles internal and external customers with kindness and respect in keeping with the values of the organization.
