We are seeking a Case Management Registered Nurse who would like to work in an environment where casual work attire is welcomed and a great attitude is a must! This position is based out of our Kapolei office.

Some Responsibilities Include

Assist with coordination of care with HMSA members

Must have excellent listening skills and be able to use critical thinking skills to quickly determine greatest need or identify gaps in care or barriers for members telephonically

Ensure continuity of care for assigned participants to avoid gaps in care

Minimum Qualifications

Hawaii Registered Nurse license with three years of experience providing direct patient care. Other experience outside hospital, traditional health care setting or Case Management Certification is a plus

Must be eligible to obtain multi-state/compact state licensure

Strong clinical skills with a basic knowledge of physiology, medications and chronic diseases
