Provides psychosocial assessment, screening, counseling, and crisis intervention services on consultation basis to patients and families, utilizing a variety of treatment modalities, including individual, family, group and specialized counseling. Works collaboratively with the multidisciplinary team to determine the need for services; to effect positive patient and family outcomes; and, to assist in securing community resources to promote continuum of care. Graduate of a Master's degree program in social work. Licensed as a social worker in the State of Maryland. One year experience in social work or related capacity. Experience in some area of specialty. Excellent verbal and written communication, psychosocial assessment skills, advocacy skills. Knowledge of personal computer and/or hospital information systems preferred, or ability to access data from relevant computer systems. Advocates for the patient and family throughout the entire episode of care. Maintains availability to the patient/family as a resource to facilitate communication among providers and to monitor services rendered. Performs other duties as assigned.
