Adult Day Care program for Mentally ill adults,seeks Case Manager.
Case Manager will have primary responsibility for a caseload as well as

• service coordination,
• provision or arrangement of services needed,
• personal advocacy, and development,
• review and updating of individual treatment and service plans.
• Client counseling in group and individual settings

Requirements

Must also possess a Masters degree in a human services field, or a Bachelors degree and two years experience in providing human services.Also acceptable is five years of human service experience, or possess a relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, bachelor's degree in social work, certified rehabilitation counselor, certified alcohol and drug counselor, bachelor's degree in rehabilitation, or bachelor of science in nursing.
