Job Details
Provides nursing care, diabetes education, and diabetes management to patients. Participates as a member of the health care team and as directed by referring physicians to meet patient needs and provide continuity of care. Serves as a clinical resource and provides health educational services for patients, families and staff to care for disease or medical condition in order to expedite discharge and decrease readmission/return visits. Coordinates and participates in health fairs and/or educational programs. REQUIREMENTS:Graduate of an approved nursing school/program. Qualified by education, training or experience to work with the school-age, adolescent, adult and/or geriatric patient population as specialty assignment dictates. Twenty-four (24) months clinical/patient care experience with the ability to apply nursing knowledge and sills required to care for diabetic patients. Must have one (1) year of study completed for CDE (Certified Diabetic Educator). Knowledge of diabetes management and education principles required. Excellent communications skills required. Demonstrated ability to work both independently and as part of a team, and to make independent judgments/decisions regarding patient care, within scope of practice. Problem-solving skills required.
Current RN licensure to work in state ofTennessee. Current CPR/BLS certification. Certificate as a Diabetes Educator (CDE) required within one (1) year of assignment. PHYSICAL DEMANDS: Prolonged standing and walking; lifting, turning patients and equipment; repeated bending, squatting and stooping; exposure to infectious agents. Ability to conduct physical rounds to nursing/patient care units required. Visual acuity is needed to assess color changes, to verify accuracy of written materials, and to administer and accurately prepare medications. Heavy Work- Exerting up to 150 pounds of force occasionally, and/or up to 75 pounds of force frequently, and/or up to 30 pounds of force constantly to move objects. (May be further defined as "with or without assistance.")
Graduation from Approved/Accredited Nursing School
TN State RN Licensure Required
1-2 years experience is required
