Job Description
- FULL-TIME STARTING ANNUAL SALARY $35,000 PLUS BENEFITS****
- Provides primary, episodic, brief mental health intervention in a Federally Qualified Health Service setting for the purpose of improving the quality of life for the adults, children, and families served.
DUTIES AND RESPONSIBILITIES (Includes But Not Limited To)
Provides primary screening and assessment to persons referred by medical and other health providers in a health service setting. Serves as liaison between behavioral health and medical health components of the health center. Gathers information by phone or in-person about the potential consumer’s symptoms, needs, strengths, and preferences to assist in decision-making. Facilitates referral of consumers into the behavioral health program of the health center or to another community provider.
Keeps abreast of community resources and interacts with consumers in a respectful and professional manner.
Provides behavioral/mental health information/consultation to medical personnel in the health center. Coordinates activities of all mental health service providers in the clinic to ensure continuity of care.
Performs intake interviews, which includes the gathering of information about the consumer’s symptoms, behavior, functioning, history, needs, goals and preferences. Obtains necessary releases and consents and advises the consumers about their rights and responsibilities. Consults with other program staff, as needed, to facilitate referrals.
Carries a differentiated caseloadinvolving highly complex and difficult situations requiring intensive clinical services and extensive professional diagnostic and treatment experience.
Provides crisis services, individual therapy, family therapy, group therapy and case management, as indicated by consumer need and according to a treatment plan.
Provides clinical supervision and technical guidance to other professional staff.
Participates in staff consultations for the purpose of case presentation and diagnostic and treatment planning for consumers.
Participates in weekly, mandatory program staff meeting.
Maintains clinical records in a timely manner as required by health center, agency, state or federal funding sources and accreditation authorities. Completes all required forms and maintains same in a manner consistent with requirements.
Full time employees spend at least 55% of work hours providing direct services to consumers. This includes flexibility in scheduling to accommodate consumer and family needs for evening appointments.
Participates in trainings/workshops related to primary intervention and therapy, including learning evidence-based practices and expanding knowledge base in child mental health.
Performs other duties as required and/or assigned by program manager and/or executive staff.
Skills/Requirements
Requires a Master’s degree and clinical licensure in the State ofOklahomain the field of social work. Must comply with all requirements to maintain professional licensure.
Must possess and maintain current CPR, First Aid andCAPEcertifications. If not current upon hire, must obtain within 30 days of employment with NorthCare. Local and out-of-state travel is required. Must possess and provide verification of a validOklahomadriver’s license during employment with NorthCare. Must maintain and provide verification of current automobile liability insurance including uninsured motorist coverage. Must have and/or complete Core Level Co-Occurring Training and Trauma Informed Training viaNetSmartUniversitywithin 60 days of employment with NorthCare. Must have TB test within 12 months prior to employment with NorthCare, or obtain within 30 days of employment with NorthCare.
Must display proficiency in the MS Word processing software and intermediate general computer skills. Excellent written and oral skills are essential, along with strong organizational skills. Must be able to move about the facilities freely and sit for extended periods. Must demonstrate good interpersonal skills and the ability to effectively communicate verbally with English-speaking consumers.
