Integrated Care Coordinator Case Management/Utilization - Pool

Hours Per Week: as needed
Location: Waukesha, WI
Detailed Work Schedule: Scheduled as needed. The position will include day shifts with varied start times, as well as 2 weekend rotations per month, and 2 holidays per year.

Job Description

Job Summary: The Care Coordinator works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in assessing, planning, facilitating and evaluating timely coordination of quality, cost-effective care across the continuum. She/he works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.

Qualifications

Minimum of three to five (3 to 5) years experience in health care. Hospital experience with previous care coordination or case management experience a plus.

Licensure required as a Registered Nurse by the State of Wisconsin.

Minimum Education

RN with Assoc degree in Nursing or equivalent experience required.

Preferred Education

Bachelors Degree in Nursing.

Location of Position: Coverage at both Waukesha Memorial Hospital and Oconomowoc Memorial Hospital.
