Job Description

Job Summary: The RN Case Manager/WOCN works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care environment. The RN Case Manager assesses plans, facilitates and evaluates timely coordination of quality, cost-effective care and works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.

Qualifications

Registered Nurse (RN), State of Wisconsin Board of Nursing; WOCN Required; Valid WI Drivers License with good driving record. Associate Degree in Nursing from an accredited school of professional nursing. 2 years nursing experience in a medical or surgical environment. Bachelor's Degree in Nursing from an accredited school of professional nursing with 2 years nursing experience in home care or home hospice setting preferred.
