Summary Of Duties

Responsible for case management of hospice patients. Must be organized and able to manage time effectively.

Education

Current state license as RN.

Experience

Minimum of 1 years nursing experience. 3 years preferred. Hospice and/or home health experience preferred.

Skills

Excellent oral and written communication skills. Must have good verbal and written skills. Must possess good assessment and documentation skills.
