Position Summary

The Case Manager is responsible for providing high quality patient care, displaying good skills in assessment, psychodiagnosis, treatment planning, group therapy, individual therapy, family therapy, and discharge planning. The Case Manager is expected to function as a member of the multidisciplinary team and participate in the Unit’s community relations program. Reports to Program Director.

Qualifications

Good overall clinical skills, ability to work productively and effectively, ability to function well as a member of a multi-disciplinary team, and ability to represent the program well in interactions both inside and outside the client hospital.

EDUCATION/EXPERIENCE

Must be must be licensed RN or LVN

At Least One year of experience
