Butler Behavioral Health Services in partnership with Ohio Department of Mental Health is recruiting healthcare professionals to become part of one of the first 'Health Homes' in Ohio. Health Home staff will work as a team and in conjunction with each client's primary health provider to improve overall health outcomes. Programming will utilize best practices in integrated care of chronic conditions such as diabetes, COPD, obesity, hypertension among youth and adults with mental disorders. Programming will include proactive interventions to promote wellness and healthy lifestyle choices. Each team will have a Team Leader, Care Coordinator and Qualified Health Home Specialists who will work together with clients to promote self-efficacy and improve health care coordination among behavioral health and primary health providers. This is an exciting opportunity to help BBHS build a true 'Person Centered Health Home', creatively innovate new approaches and change healthcare practices.

Qualifications

BA in social work, psychology or related field; LPN with behavioral
health experience; QMHS with experience providing CPST in community mental health
setting.
