Knowledge Of Disease Management

Knowledge and experience in Patient Coaching
Knowledge and experience in Care Coordination
At least two years home health experience
Must possess strong interpersonal, communication and coaching skills with ability to adapt to a changing workload
Must work well in a team environment
Must be computer proficient
Experience with Point of Care Documentation a plus
